Background:

  • Tuberculosis (TB) is a significant global health issue, with extrapulmonary manifestations posing diagnostic challenges.
  • While extrapulmonary TB is less common in adolescents, it must be considered in complex cases.
  • Bacillus Calmette-Guérin (BCG) vaccination does not preclude the possibility of tuberculosis.

Observation:

  • A 14-year-old girl presented with abdominal pain, weight loss, and fatigue.
  • Imaging and elevated CA-125 suggested pelvic malignancy.
  • Laparotomy revealed a pelvic mass, initially presumed malignant.

Findings:

  • Histopathology showed noncaseous granulomata, raising suspicion for TB.
  • Despite initial misleading results, persistent clinical suspicion led to the diagnosis of pelvic tuberculosis.
  • The causative agent, *Mycobacterium tuberculosis*, was ultimately identified.

Implications:

  • Pelvic tuberculosis can masquerade as malignancy, complicating diagnosis in young patients.
  • Early and accurate diagnosis is vital to prevent aggressive surgical interventions and preserve reproductive health.
  • Increased awareness of extrapulmonary TB in adolescents is necessary for appropriate differential diagnosis.
