    Summary

    Idiopathic calcium stone formers lack a normal daily rhythm in urinary hydroxyproline (a bone collagen breakdown marker). This suggests increased bone turnover may contribute to hypercalciuria in these patients.

    Background:

    • Idiopathic calcium urolithiasis is a common condition.
    • Hypercalciuria is a major risk factor for calcium nephrolithiasis.
    • The precise mechanisms causing hypercalciuria in idiopathic calcium stone formers remain incompletely understood.

    Purpose of the Study:

    • To investigate the circadian rhythm of urinary total hydroxyproline (THP) excretion.
    • To determine if enhanced bone resorption contributes to hypercalciuria in idiopathic calcium stone formers.

    Main Methods:

    • Matched groups of ten male idiopathic calcium stone formers and ten normal subjects were studied.
    • Eight-hour urine collections were performed over a 24-hour period.
    • Urinary total hydroxyproline (THP) and creatinine levels were measured.

    Main Results:

    • Normal subjects exhibited a significant circadian rhythm in THP excretion, with higher levels during the night.
    • Idiopathic calcium stone formers showed no significant circadian rhythm in THP excretion.
    • Stone formers had significantly higher THP excretion during the 8 AM to 4 PM period compared to normal subjects.

    Conclusions:

    • The absence of a normal circadian rhythm in THP excretion in stone formers suggests altered bone metabolism.
    • Increased bone turnover may be a contributing factor to hypercalciuria in idiopathic calcium stone formers.
    • Further research is warranted to elucidate the role of bone metabolism in calcium nephrolithiasis.
