Background:

  • Overactive bladder (OAB) is a common condition affecting bladder control.
  • Retrograde ejaculation (RE) is a condition where semen enters the bladder during orgasm.
  • The interplay between OAB and RE is not well understood.

Purpose of the Study:

  • To investigate the impact of OAB on patients diagnosed with RE.
  • To explore the potential for therapeutic intervention in patients with coexisting OAB and RE.

Main Methods:

  • Retrospective analysis of a prospectively collected database from five urology polyclinics.
  • Utilized International Index of Erectile Function-5 (IIEF-5), Overactive Bladder 8-Question Awareness Tool (OAB-V8), urodynamics, and semen analysis.
  • Compared 47 non/minimal symptomatic RE patients, 73 symptomatic RE patients with OAB treated with trospium, and 37 OAB-only control patients.

Main Results:

  • No significant differences in OAB examination or urodynamic values between symptomatic and non/minimal symptomatic RE groups.
  • Sperm and fructose were detected in post-ejaculation urine in the symptomatic RE with OAB group.
  • Three months of trospium treatment significantly increased sperm count and ejaculate volume in symptomatic RE with OAB patients, but not in the control group.

Conclusions:

  • This study is the first to demonstrate the coexistence of RE and OAB using urodynamics.
  • Trospium treatment shows potential efficacy in improving reproductive parameters in selected patients with coexisting RE and OAB.
