    Summary

    Continuous-infusion cisplatin is well-tolerated at 25 mg/m2/day, showing efficacy in patients who previously failed bolus cisplatin chemotherapy. Lower doses demonstrated no neurotoxicity or nephrotoxicity.

    Background:

    • Cisplatin is a widely used chemotherapy agent.
    • Continuous infusion administration may alter its toxicity and efficacy profile.
    • Previous studies have explored various cisplatin dosing and administration schedules.

    Purpose of the Study:

    • To evaluate the safety and efficacy of continuous infusion cisplatin.
    • To determine the maximum tolerated dose (MTD) of continuous infusion cisplatin.
    • To assess response rates in patients previously treated with bolus cisplatin.

    Main Methods:

    • Twenty-one patients received continuous infusion cisplatin over 5 days at doses of 20, 25, or 30 mg/m2/day.
    • Patients were monitored for neurotoxicity, nephrotoxicity, and hematologic toxicity.
    • Response evaluation was performed on 19 evaluable patients.

    Main Results:

    • Continuous infusion cisplatin at 30 mg/m2/day was associated with neuropathy and elevated creatinine in some patients.
    • No neurotoxicity or nephrotoxicity was observed at lower doses (20-25 mg/m2/day).
    • Partial or minimal responses were observed in 57% of evaluable patients, including some who had failed prior bolus cisplatin therapy.

    Conclusions:

    • Continuous infusion cisplatin is well-tolerated at 25 mg/m2/day.
    • This administration method may offer a viable treatment option for patients refractory to bolus cisplatin.
    • Further investigation into optimal dosing and patient selection is warranted.
