    Summary

Technetium Tc-99m dimercaptosuccinic acid (DMSA) imaging may overestimate kidney function in cases of obstruction. This study in rats demonstrated a significant overestimation of at least 17% in obstructed kidneys.

    Area of Science:

    • Nephrology
    • Nuclear Medicine
    • Surgical Research

    Background:

    • Technetium Tc-99m dimercaptosuccinic acid (DMSA) scintigraphy is used to assess kidney function.
    • Overestimation of renal function in obstructed kidneys via DMSA imaging has been suggested but not well-quantified.
    • The clinical significance of potential overestimation requires further documentation.

    Purpose of the Study:

    • To quantify the degree of overestimation of kidney function by Tc-99m DMSA imaging in a model of partial ureteral obstruction.
    • To investigate the clinical relevance of functional overestimation in obstructed kidneys.

    Main Methods:

    • A rat model was used to create partial obstruction of one ureter.
    • Relative Tc-99m DMSA uptake was measured 4 hours after intravenous injection.

    Related Experiment Videos

  • Kidney function was assessed by comparing DMSA uptake with direct measurement after puncturing the obstructed kidney.
  • Main Results:

    • Partial ureteral obstruction led to an overestimation of kidney function.
    • The function of the obstructed kidney was overestimated by at least 17% based on Tc-99m DMSA uptake.
    • This indicates a significant discrepancy between imaging-based and actual renal function.

    Conclusions:

    • Tc-99m DMSA imaging can significantly overestimate the function of an obstructed kidney.
    • The findings highlight the need for careful interpretation of DMSA scans in patients with suspected or known urinary tract obstruction.
    • Further clinical studies are warranted to confirm these findings in human subjects.
