Purpose of the Study:

  • To assess the feasibility of computed tomography enterography (CTE) for quantitative evaluation of Crohn's disease activity.
  • To identify imaging features on CTE that correlate with disease severity.

Main Methods:

  • Retrospective analysis of 49 Crohn's disease patients with clinical, enteroscopy, and CTE data.
  • Patients were categorized into inactive, mild, and moderate-severe groups based on the simplified endoscopic activity score for Crohn's disease (SES-CD).
  • Bowel wall thickening, mural hyperenhancement, and ΔCT values were analyzed and correlated with SES-CD, CRP, and ESR.

Main Results:

  • Significant differences in bowel wall thickening, mural hyperenhancement, and ΔCT values were observed across disease activity groups (P < 0.001).
  • These imaging parameters showed moderate correlations with SES-CD (r = 0.533–0.585, P < 0.001).
  • Bowel wall thickening, mural hyperenhancement, and ΔCT values were significantly different among inactive, mild, and moderate-severe CD groups.

Conclusions:

  • Mural hyperenhancement in the portal vein period, bowel wall thickening, and ΔCT value are accurate quantitative measures of CD activity.
  • These CTE-derived parameters show potential as visual biomarkers for assessing Crohn's disease lesions.
