Background:

  • Rhabdomyosarcoma of the mastoid is a rare malignancy.
  • It can be misdiagnosed as infectious mastoiditis due to similar clinical and imaging findings.
  • Early differentiation is crucial for timely and appropriate treatment.

Observation:

  • A systematic review and case report identified 43 mastoid rhabdomyosarcoma cases.
  • Compared to infectious mastoiditis, rhabdomyosarcoma presented more often with a mass (86%) and cranial nerve dysfunction (83.7%).
  • Infectious mastoiditis was more commonly associated with fever (72.4%), pain (48.2%), and younger age (4.4 years vs. 6.1 years).

Findings:

  • Distinct clinical features differentiate mastoid rhabdomyosarcoma from infectious mastoiditis.
  • Rhabdomyosarcoma patients had a median survival of 7.1 months, with 41.7% alive at reporting.
  • The average age for rhabdomyosarcoma was 6.1 years, compared to 4.4 years for mastoiditis.

Implications:

  • Identifying these unique features can improve diagnostic accuracy for mastoid masses.
  • Earlier diagnosis of rhabdomyosarcoma can lead to prompt initiation of cancer-directed therapy.
  • Distinguishing between these conditions prevents treatment delays and improves patient outcomes.
