Tuberculous sclerokeratitis, a rare eye condition, can mimic other diseases by presenting with hypopyon. Prompt diagnosis and antitubercular treatment are crucial for managing this ocular manifestation of tuberculosis.

Background:

Tuberculous sclerokeratitis is an uncommon ocular condition.
It can present with varied clinical features, challenging diagnosis.

Observation:

A 23-year-old female presented with nodular scleritis, peripheral corneal opacities, and severe anterior uveitis with hypopyon.
Diagnostic tests revealed positive tuberculosis markers (Mantoux, interferon gamma release assay) and hilar lymphadenopathy.

Findings:

Aqueous aspirate for Mycobacterium tuberculosis genome was negative.
The patient showed significant improvement following antitubercular therapy and oral corticosteroids.

Implications:

This case highlights the diagnostic challenges of tuberculous sclerokeratitis, especially when presenting with hypopyon.
Early recognition and appropriate treatment are vital for favorable visual outcomes in tuberculous sclerokeratitis.
