An Unusual Presentation in Urinothorax.

    Summary

    Urinothorax, the presence of urine in the chest cavity, can result from urinary tract injury. A high pleural fluid to serum creatinine ratio confirmed this rare diagnosis in a patient with a history of ureteric injury.

    • Urinothorax is a rare condition characterized by urine accumulation in the pleural space.
    • It typically arises secondary to urinary tract leakage, often forming a urinoma in the retroperitoneum first.

    • A 35-year-old female presented with shortness of breath and right-sided chest pain.
    • Imaging revealed a right-sided pleural effusion.
    • The patient had a history of left ovarian cystectomy and a subsequent left ureteric injury requiring stent placement.

    Findings:

    • Urinothorax was suspected due to the patient's history of ureteric injury.
    • Diagnostic confirmation was achieved by measuring the pleural fluid to serum creatinine ratio.
