Summary

Autophagy protects against vascular calcification in chronic kidney disease. Inducing autophagy with rapamycin reduced calcification and improved survival in mice, suggesting a therapeutic target for cardiovascular complications.

Background:

  • Uremic vascular media calcification is a major contributor to morbidity and mortality in patients with kidney disease.

Purpose of the Study:

  • To investigate the role of autophagy in the development of uremic vascular media calcification.
  • To explore the therapeutic potential of modulating autophagy in this condition.

Main Methods:

  • Utilized a high-phosphate diet (HPD) model in DBA/2 mice to induce vascular calcification.
  • Employed an in vitro model using mouse vascular smooth muscle cells (MOVAS) under calcifying conditions.
  • Administered autophagy modulators: 3-methyladenine (3-MA), bafilomycin (inhibitors), and rapamycin (inducer).

Main Results:

  • HPD induced severe vascular media calcification in mice and MOVAS.
  • Autophagy levels increased significantly in calcifying conditions, both in vivo and in vitro.
  • Inhibiting autophagy with 3-MA or bafilomycin exacerbated calcification, while rapamycin treatment reduced calcification and improved survival.
  • Rapamycin normalized osteogenic markers (Runx2) and restored smooth muscle markers (α-smooth muscle actin, Sm22α).

Conclusions:

  • Autophagy acts as an endogenous protective mechanism against vascular smooth muscle cell calcification in uremia.
  • Inducing autophagy via rapamycin demonstrates a protective effect against uremic media calcification and associated mortality.
  • Targeting autophagy may offer a novel therapeutic strategy to mitigate cardiovascular complications in chronic kidney disease.
