Background:

  • While often attributed to disease-modifying anti-rheumatic drugs (DMARDs), other etiologies must be considered.

Observation:

  • This article presents two distinct cases of inflammatory arthritis patients with deranged serum liver enzymes.
  • In these cases, the abnormal LFTs were found to be caused by hereditary hemochromatosis and acute hepatitis E infection, respectively.

Findings:

  • Hereditary hemochromatosis, a genetic disorder causing iron overload, can manifest with abnormal liver function.
  • Acute hepatitis E infection is another critical, albeit less common, cause of liver enzyme derangement in this patient population.

Implications:

  • Accurate identification of the underlying cause of abnormal LFTs is crucial for appropriate patient management.
  • Timely and specific treatment for conditions like hereditary hemochromatosis or hepatitis E can significantly improve patient prognoses and prevent long-term complications.
