    Context:

    • Objective identification of efferent cortico-spinal pathway lesions is challenging.
    • Existing methods like transcutaneous electrical stimulation are limited.

    • To evaluate the utility of LL EMG responses in detecting lesions of the efferent cortico-spinal pathways.
    • To assess the combined diagnostic value of LL responses and sensory evoked potentials (SEP).

    • LL EMG responses from the anterior tibial muscle were recorded in 135 patients with spinal or cerebral lesions.
    • Significant LL delays were observed in patients with spinal and cerebral lesions on the affected side.
    • Combining SEP and LL methods increased pathological finding rates to approximately 65%.
