Background:

Clinically complex pediatric patients face significant challenges during hospital-to-home transitions.
Existing nurse-led discharge services (patient navigation programs) aim to mitigate these challenges.

Purpose of the Study:

To integrate pharmacy discharge services into an established nurse-led patient navigation program.
To enhance the care transition process for clinically complex pediatric patients.

Main Methods:

Pharmacists resolved medication discrepancies, provided discharge counseling, and conducted post-discharge follow-up calls.
Data collected included patient demographics, diagnoses, medications, and pharmacist interventions.
Program utilization and financial benefits were assessed.

Main Results:

The program received 321 referrals; 56 (17%) patients received pharmacy discharge counseling.
Pharmacists identified 168 interventions, with 93.5% accepted or informational.
Estimated cost savings of $22,308 were achieved in the first 5 months.

Conclusions:

A collaborative partnership between nurses and pharmacists effectively facilitated the discharge of complex pediatric patients.
The integrated service demonstrated positive impacts on patient care and financial outcomes.
