  • Positron emission tomography-CT indicated increased metabolic activity in the nodule.

  • Pathological examination revealed multifocal thymic cysts with cholesterol granuloma.
  • Microscopy showed cysts filled with cholesterol clefts and a nodule composed of granuloma with cholesterol clefts.

  • This case expands the understanding of rare thymic pathologies.
  • It underscores the importance of integrating imaging and histopathology for diagnosing complex mediastinal lesions.
