    Area of Science:

    • Immunology
    • Infectious Diseases
    • Gastroenterology

    Background:

    • The case involves a young homosexual male with a history of Hepatitis B, Syphilis (Lues), Gonorrhea, ascending myelitis, and lymphadenitis.
    • The patient presented with symptoms suggestive of chronic-inflammatory bowel disease, initially considered Crohn's disease.

    Observation:

    • The patient experienced two episodes of acute illness, both treated with Methylprednisolone.
    • Despite initial treatment, the patient's condition deteriorated.

    Findings:

    • The chronic-inflammatory bowel disease symptoms were attributed to a cytomegalovirus (CMV) infection.
    • The CMV infection was associated with acquired immune deficiency syndrome (AIDS).

    Implications:

    • This case highlights the importance of considering opportunistic infections like CMV in patients with complex medical histories and compromised immune systems.
    • Accurate diagnosis of the underlying cause of gastrointestinal symptoms is crucial for appropriate management and preventing severe outcomes.
    • The differential diagnosis for inflammatory bowel disease should include infectious etiologies, particularly in immunocompromised individuals.
