Summary

Stress significantly impacts transient global amnesia (TGA) pathogenesis. Elevated cortisol during TGA correlates with depression and anxiety, and TGA patients show heightened stress responses, supporting stress as a key factor.

Background:

  • Transient global amnesia (TGA) is characterized by temporary memory loss.
  • Stress-related inhibition of hippocampal memory formation is a hypothesized cause of TGA.
  • TGA episodes often follow stressful events.

Purpose of the Study:

  • To investigate the role of cortisol and stress in TGA pathogenesis.
  • To compare cortisol levels and stress responses in TGA patients and healthy controls.

Main Methods:

  • Salivary cortisol was measured during acute TGA and during experimental stress exposure (cold pressor test) in 14 TGA patients and 20 controls.
  • Psychiatric comorbidity, depression, trait anxiety, and chronic stress were assessed.
  • Cortisol day-profiles were analyzed.

Main Results:

  • TGA patients had 3-fold higher salivary cortisol during TGA episodes compared to controls.
  • TGA patients exhibited a significantly higher cortisol increase during experimental stress.
  • TGA patients reported higher depressive symptomatology and chronic stress levels.
  • Cortisol increase during TGA correlated significantly with depression and anxiety scores.

Conclusions:

  • Enhanced cortisol secretion in acute TGA patients supports the role of stress in TGA pathogenesis.
  • Persisting hyperreactivity to stress in the subacute phase further implicates stress.
  • Findings suggest stress is a significant factor in TGA development.
