Background:

Cystosarcoma phyllodes (now often referred to as phyllodes tumors) are rare breast tumors with unpredictable behavior.
Accurate prediction of malignant potential and patient outcomes is crucial for effective management.

Purpose of the Study:

To identify histologic features that correlate with adverse outcomes in cystosarcoma phyllodes.
To evaluate the prognostic significance of stromal overgrowth in these tumors.

Main Methods:

Retrospective review of 26 cystosarcoma phyllodes cases diagnosed at a single institution.
Analysis of clinicopathologic factors including tumor size, stromal overgrowth, mitotic rate, and necrosis.
Correlation of histologic findings with local recurrence, metastasis, and tumor-related death.

Main Results:

Seven of 26 tumors were associated with patient death.
Five patients developed metastasis, all of whom died.
Stromal overgrowth was present in 8 cases and significantly correlated with both metastatic spread (P=0.0014) and tumor death (P=0.02).

Conclusions:

Stromal overgrowth is a significant histologic indicator of malignant behavior in cystosarcoma phyllodes.
The presence of stromal overgrowth predicts a higher risk of metastasis and mortality.
This finding can aid clinicians in risk stratification and treatment planning for patients with phyllodes tumors.
