    Fibrolamellar hepatoma (FLH) is a rare liver cancer predominantly affecting young adults and distinct from hepatocellular hepatoma (HCC). Early diagnosis of FLH offers better resectability and survival rates compared to HCC.

    • The patient presented with polygonal eosinophilic tumor cells and characteristic lamellar fibrous stroma.
    • A review of approximately 140 published FLH cases is included.

    Findings:

    • Histological differentiation from focal nodular hyperplasia is a diagnostic challenge.
    • Specific biomarkers like neurotensin, vitamin B12 binding capacity, and copper stains may aid diagnosis.
    • FLH exhibits distinct clinical characteristics and disease progression compared to HCC.

    Implications:

    • Early and accurate diagnosis of FLH is crucial for patient outcomes.
    • FLH demonstrates good resectability when detected early.
    • Timely intervention for FLH can lead to a significantly better survival rate than HCC.
