    Summary

    Emergency colectomy for severe ulcerative colitis complications (UC) can be life-saving. While colectomy offers a chance for recovery, preserving the rectal stump for ileo-proctostomy may benefit select patients with UC.

    Background:

    • Ulcerative colitis (UC) can present with acute, life-threatening complications requiring surgical intervention.
    • Emergency total colectomy is a critical treatment for severe UC complications like perforation, toxic megacolon, and massive bleeding.

    Observation:

    • Seven patients with ulcerative colitis experienced acute complications including colonic perforation, toxic megacolon, and massive enterorragia over 12 years.
    • All patients underwent emergency total colectomy, resulting in a 43% mortality rate (3 deaths).
    • Two of the deceased patients required a second surgery to remove the rectal stump.

    Findings:

    • Three to four recovered patients underwent ileo-proctostomy 7-15 months post-colectomy after rectal lesion healing.
    • The study discusses surgical indications and procedures for UC complications, evaluating the benefits and limitations of rectal stump preservation.

    Implications:

    • An eclectic surgical approach is recommended, tailoring treatment to individual patient pathology, age, and overall condition.
    • Careful consideration of rectal stump preservation in ileo-proctostomy can be a viable option for select ulcerative colitis patients post-colectomy.
