Bleeding in renal failure: is von Willebrand factor implicated?

G Remuzzi, M Livio, M C Roncaglioni

    British Medical Journal
    |August 6, 1977
    PubMed
    Summary

Patients with chronic renal failure (CRF) often experience bleeding issues. However, this study found that factor VIII complex components (VIIIC, VIIIRA, VIIIVWF) are typically normal or increased in CRF patients, suggesting bleeding isn't due to these factors.

    Area of Science:

    • Hematology
    • Nephrology
    • Clinical Pathology

    Background:

    • Bleeding tendency is common in patients with chronic renal failure (CRF).
    • The role of factor VIII complex components in CRF-related bleeding is not fully understood.

    Purpose of the Study:

    • To investigate the concentrations of factor VIII procoagulant activity (VIIIC), factor VIII-related antigen (VIIIRA), and factor VIII-von Willebrand activity (VIIIVWF) in patients with CRF.
    • To determine if abnormalities in these factor VIII components contribute to the bleeding tendency in CRF.

    Main Methods:

    • Analysis of predialysis plasma samples from 11 patients with CRF.
    • Measurement of VIIIC, VIIIRA, and VIIIVWF concentrations.
    • Assessment of bleeding time and platelet retention to glass beads.

    Related Experiment Videos

    Main Results:

    • 10 out of 11 CRF patients showed normal or increased concentrations of all three factor VIII components.
    • All patients exhibited prolonged bleeding times (>15 minutes) and low platelet retention (<40%).
    • One patient with reduced factor VIII components was diagnosed with von Willebrand's disease.

    Conclusions:

    • The common bleeding tendency in CRF is not necessarily linked to defective plasma factor VIII-related activities.
    • Factor VIII complex component levels are generally not reduced in CRF patients with bleeding issues.
    • Further research is needed to identify the specific causes of bleeding in CRF.
