Background:

  • Non-small cell lung cancer (NSCLC) is a leading cause of cancer mortality worldwide.
  • Accurate prognostic markers are crucial for tailoring treatment strategies in NSCLC.
  • 18F-FDG PET/CT offers quantitative metabolic information that may correlate with patient outcomes.

Purpose of the Study:

  • To evaluate the prognostic significance of quantitative [18F]fluorodeoxyglucose (FDG) positron emission tomography (PET)/computed tomography (CT) parameters for overall survival (OS) in NSCLC patients.
  • To identify specific metabolic tumor volume (MTV) and total lesion glycolysis (TLG) metrics with prognostic value.

Main Methods:

  • Retrospective analysis of 193 NSCLC patients who underwent baseline 18F-FDG PET/CT before treatment.
  • Multivariate Cox regression analysis adjusted for clinical and demographic factors.
  • Kaplan-Meier curves were used to assess associations between PET/CT parameters and OS, grouped by tertiles.

Main Results:

  • Significant associations between OS and MTVPETVCAR, MTV2.5, MTV25%, MTV42%, and TLGPETVCAR were observed.
  • No significant associations were found between OS and standard uptake values (SUV, SUL), or certain MTV and TLG metrics (MTV50%, MTV75%, TLG2.5).
  • Subgroup analyses revealed significant associations in adenocarcinoma and squamous cell carcinoma groups, and in early-stage disease.

Conclusions:

  • Quantitative 18F-FDG PET/CT parameters, including MTVPETVCAR, MTV2.5, MTV25%, MTV42%, and TLGPETVCAR, demonstrate prognostic value for OS in NSCLC.
  • These metabolic parameters, particularly MTV metrics, can aid in predicting NSCLC prognosis.
  • Selection of appropriate metabolic parameters from 18F-FDG PET/CT is valuable for patient outcome prediction.
