Summary

This study found that while 18F-Choline PET/CT and MRI parameters for aggressive prostate cancer showed some correlations, only FCH influx moderately linked to vessel permeability. Further research is needed to fully understand these relationships.

Background:

  • 18F-Choline (FCH) PET/CT uptake parameters indicate aggressive prostate cancer.
  • Multiparametric MRI (mpMRI) functional parameters also correlate with disease aggressiveness.

Purpose of the Study:

  • To investigate the relationship between metabolic parameters from FCH PET/CT and functional parameters from mpMRI in prostate cancer patients.
  • To assess the correlation between FCH uptake and MRI-derived metrics for aggressive disease.

Main Methods:

  • 14 prostate cancer patients underwent FCH PET/CT and mpMRI.
  • Tumoral regions of interest were defined on ADC maps and projected onto PET/CT and MRI images.
  • Kinetic and standardized uptake value (SUV) parameters were extracted from FCH PET/CT.
  • Apparent diffusion coefficient (ADC), Ktrans, and Ve were extracted from mpMRI.
  • Spearman's correlation coefficients were calculated to compare parameters.

Main Results:

  • The one-tissue compartment model was selected for PET kinetic analysis.
  • PET parameters like K1 and SUVmean showed high correlations among themselves.
  • No significant correlations were found between different functional MRI parameters.
  • A moderate correlation was observed between FCH influx (K1) and Ktrans (vessel permeability) from MRI (r=0.55, p=0.041).

Conclusions:

  • Significant correlations between FCH PET/CT and mpMRI metrics were limited.
  • FCH influx demonstrates a moderate association with tumor vessel permeability in prostate cancer.
  • Further investigation is warranted to elucidate the combined utility of FCH PET/CT and mpMRI.
