    Background:

    • Human parvovirus B19 is a common virus typically causing mild, self-limiting illness.
    • Arthritis and rash are known manifestations of parvovirus B19 infection, particularly in adults.
    • However, severe purpura with skin necrosis is an atypical presentation.

    Observation:

    • A 48-year-old male presented with severe symmetrical arthritis in knee and ankle joints.
    • He also exhibited an extensive purpuric rash on both legs with areas of skin necrosis.
    • No other organ systems were clinically affected.

    Findings:

    • Recent human parvovirus B19 infection was confirmed via IgM antibody detection.
    • The patient's symptoms included severe joint inflammation and significant skin manifestations.
    • Purpura and skin necrosis are not previously reported in association with human parvovirus B19.

    Implications:

    • This case expands the known clinical spectrum of human parvovirus B19 infection.
    • It underscores the importance of considering parvovirus B19 in patients with severe arthritis and purpuric rash, even with necrosis.
    • Further research may be warranted to understand the mechanisms behind this severe presentation.
