Background:

  • Pleomorphic dermal sarcoma (PDS) is a rare, aggressive mesenchymal neoplasm.
  • Distinguishing PDS from atypical fibroxanthoma (AFX) is challenging due to overlapping clinical and histological features.

Observation:

  • A 77-year-old male presented with a rapidly growing, large nodular scalp lesion.
  • The lesion measured 3.3 cm and was confirmed as invasive pleomorphic dermal sarcoma on pathology.

Findings:

  • Histological examination revealed features consistent with pleomorphic dermal sarcoma.
  • Complete surgical excision with 2 cm margins was achieved, followed by successful graft healing.

Implications:

  • This case underscores the importance of accurate diagnosis of PDS.
  • Effective management involves complete surgical resection for favorable outcomes.
