Gliosarcoma arising from oligodendroglioma (Oligosarcoma): A case report with genetic analyses

Background:

• Gliosarcomas are rare WHO grade IV glioblastoma variants with glial and sarcomatous elements.

Observation:

• A 53-year-old man with a history of oligodendroglioma (WHO grade II) presented with tumor recurrence.
• Histopathology of the recurrent tumor revealed classical oligodendroglioma with emergent sarcomatous features.
• Both primary and recurrent tumors demonstrated 1p/19q co-deletion and isocitrate dehydrogenase 1 (IDH1) gene mutation.

Findings:

• The recurrent tumor exhibited characteristics of both oligodendroglioma and sarcoma.
• Genetic analyses, including 1p/19q co-deletion and IDH1 mutation status, were performed.
• The sarcomatous component was identified as a metaplastic change within the oligodendroglial element.

Implications:

• This case provides evidence for metaplasia as the mechanism for sarcomatous component development in oligosarcoma.
• Genetic markers (1p/19q co-deletion, IDH1 mutation) are key to identifying the oligodendroglial origin.
