    Summary

    Alzheimer's disease patients showed Lewy bodies and neurofilament abnormalities in the brain. These findings suggest early changes in brain cells that may precede Lewy body formation in Alzheimer's disease.

    Background:

    • Investigating the neuropathological underpinnings of clinical Alzheimer's disease (AD).
    • Examining the co-occurrence of Lewy bodies and Alzheimer's pathology.

    Observation:

    • Postmortem examination of three patients with clinical AD revealed Lewy-bodies and Lewy-like bodies in the cerebral cortex and brainstem nuclei.
    • Neuritic plaques were present in neocortical areas, but neurofibrillary tangles were absent.
    • Neuronal cell loss was significant in the ventral tegmental area and basal nucleus of Meynert.

    Findings:

    • The distribution of cortical neuronal inclusions correlated with dopaminergic projections.
    • Immunohistochemistry showed Lewy-bodies stained positive for phosphorylated neurofilaments.
    • Neurons without inclusions also showed positive staining, indicating an early neurofilament abnormality.
