Austrian syndrome, a rare Streptococcus pneumoniae infection triad of pneumonia, meningitis, and endocarditis, was complicated by septic arthritis in a unique case. This report details this rare presentation of invasive pneumococcal disease.

Background:

Austrian syndrome is a rare clinical triad of invasive Streptococcus pneumoniae infection, encompassing pneumonia, meningitis, and endocarditis.
This syndrome, also known as Osler's triad, represents a severe manifestation of pneumococcal disease.

Observation:

This report presents an exceptionally rare case of Austrian syndrome.
The case was further complicated by the development of septic arthritis, adding a rheumatological dimension to the invasive infection.

Findings:

The patient presented with the classic Osler's triad (pneumonia, meningitis, endocarditis) caused by Streptococcus pneumoniae.
Septic arthritis was identified as an additional complication, highlighting the extensive dissemination of the infection.

Implications:

This case expands the known spectrum of Austrian syndrome complications.
It underscores the importance of considering septic arthritis in patients with invasive Streptococcus pneumoniae infections presenting with Osler's triad.
Early recognition and management of such complex presentations are crucial for patient outcomes.
