Background:

Information technology (IT) has expanded university teaching and learning methods.
E-learning implementation in Iranian universities requires assessing learner preparedness.
This study investigated E-learning system challenges at Tehran University of Medical Sciences.

Purpose of the Study:

To identify challenges in the E-learning system at Tehran University of Medical Sciences.
To assess learner preparation for E-learning environments.

Main Methods:

Descriptive, cross-sectional study conducted in 2016-2017.
Stratified random sampling of 300 students enrolled in E-learning courses.
Data collected via a researcher-made questionnaire and analyzed using SPSS software.

Main Results:

40% of participants faced technology access problems.
Only 26.4% of students demonstrated good E-learning system preparation.
Significant differences were observed in participants' skill and cultural challenges (P=0.01).

Conclusions:

Identifying skill, technical, and cultural challenges is essential for successful E-learning.
Establishing IT infrastructure, adopting international best practices, and fostering a supportive culture are necessary.
Familiarizing learners and educators with E-learning development and usage is vital.
