    Thrombin generation is lower in myelofibrosis patients but higher with increased platelet counts. This test confirms thrombocythemia as a risk factor for blood clots in myelofibrosis.

    Background:

    • Primary myelofibrosis (PMF) is a chronic myeloid disorder, part of Philadelphia chromosome-negative myeloproliferative neoplasms.
    • Thrombotic events are serious complications impacting patient quality of life and survival.
    • Known risk factors include age, prior thrombosis, cardiovascular risks, and JAK2 V617F mutation.

    Purpose of the Study:

    • To evaluate the utility of the thrombin generation test (TGT).
    • To assess TGT's potential in predicting thrombosis risk in PMF patients.

    Main Methods:

    • Analyzed plasma from 36 PMF patients and 20 healthy controls.
    • Measured thrombin generation using the TECHNOTHROMBIN® TGA kit and Ceveron® Alpha system.
    • Correlated TGT results with clinical and laboratory parameters.

    Main Results:

    • Endogenous thrombin potential (ETP) was significantly lower in PMF patients compared to controls (p = 0.0003).
    • Higher thrombin generation correlated with elevated platelet counts (> 400 x 10^9/L) (p = 0.04).
    • ETP was higher in earlier disease stages and lower in CALR-mutated myelofibrosis.

    Conclusions:

    • Thrombin generation in PMF is influenced by platelet count, mutation status, and disease progression.
    • The thrombin generation test supports thrombocythemia as a risk factor for thrombotic complications in PMF.
