Summary

This study shows that a comprehensive laparoscopic retroperitoneal dissection curriculum improved surgical residents' knowledge and technical skills. Simulation training successfully translated to improved operative performance and ureter identification in the operating room.

  • A curriculum combining didactic and technical skills using a pelvic model was developed.
  • Novice gynecologic surgeons (PGY 3-5) participated, undergoing pre/post-curriculum knowledge assessments and technical skills evaluations.
  • Expert feedback was provided during simulation, and participants' operative performance was video-recorded and assessed using the Objective Structured Assessment of Technical Skills (OSATS) tool.

Main Results:

  • Significant improvements were observed in both knowledge (P < .001) and technical skills (OSATS scores, P < .001) post-curriculum.
  • Intraoperative OSATS scores correlated with simulation-based scores (r = 0.53, P = .01).
  • 91% of participants successfully identified the ureter intraoperatively, and 81% reported increased comfort with the procedure.

Conclusions:

  • A comprehensive curriculum for laparoscopic retroperitoneal dissection effectively enhances cognitive knowledge and technical skills.
  • The simulation-based training demonstrated a positive translation to competent performance in the operating room.
  • Residents perceived improved skills acquisition and comfort following the curriculum.
