Background:

  • Transcatheter valve interventions, such as MitraClip, are increasingly used for severe mitral regurgitation.
  • Cerebral ischemic lesions are a significant complication, necessitating preventative strategies.
  • Cerebral protection devices aim to reduce embolic events during these procedures.

Observation:

  • An 84-year-old female patient underwent a trans-esophageal echocardiography (TEE)-guided MitraClip procedure.
  • A fibrin strand was observed on the guide catheter during the procedure.
  • A Claret CE Pro cerebral protection device was deployed in response.

Findings:

  • The fibrin strand/thrombus disappeared after device deployment and before procedure completion.
  • A large thrombus was retrieved by the filter device post-procedure.
  • Histologic analysis confirmed the retrieved material was a thrombus.

Implications:

  • This case highlights the potential role of cerebral protection devices in capturing embolic material during transcatheter mitral valve interventions.
  • The findings support the use of such devices to mitigate the risk of stroke in this patient population.
  • Further research and larger studies are warranted to establish the definitive benefit of cerebral protection devices.
