Summary

Renal hilar tumors (tumors near the renal hilum) do not show increased malignancy or recurrence risk compared to nonhilar tumors. This suggests conservative management may be appropriate for hilar renal masses.

Background:

  • Renal masses involving the hilum present unique management challenges.
  • Histopathologic outcomes for hilar versus nonhilar renal masses are not well-defined.

Purpose of the Study:

  • To compare histopathologic features and clinical outcomes of solitary cT1 renal masses with and without hilar involvement.
  • To determine if hilar location impacts malignancy risk, grade, upstaging, or recurrence.

Main Methods:

  • Retrospective analysis of a prospectively maintained database of resected cT1 renal masses.
  • Classification using the R.E.N.A.L. nephrometry score; hilar masses defined by proximity to renal artery/vein.
  • Comparison of demographic, treatment, and histopathologic characteristics; multivariate regression for upstaging and recurrence.

Main Results:

  • 1324 cT1 renal masses analyzed; 226 (17.1%) were hilar.
  • Hilar masses were larger, more complex, and more likely to undergo radical nephrectomy.
  • No significant differences in malignancy, high nuclear grade, upstaging, or recurrence between hilar and nonhilar masses.

Conclusions:

  • Hilar location of cT1 renal masses does not increase the risk of malignancy, high nuclear grade, upstaging, or recurrence.
  • These findings suggest no compelling cancer-specific rationale for radical nephrectomy in managing renal hilar tumors.
