Background:

  • Severe hypocalcemia (calcium <1.9 mmol/L) is often treated as an emergency due to perceived risks of cardiac arrest or seizures.
  • Limited evidence currently supports the immediate life-threatening nature of severe hypocalcemia.

Purpose of the Study:

  • To investigate the association between severe hypocalcemia and life-threatening cardiac arrhythmias or neurological complications.
  • To evaluate the clinical significance of severe hypocalcemia in an emergency department setting.

Main Methods:

  • A retrospective observational study conducted over two years in an Adult Emergency Department.
  • Inclusion of patients with measured protein-corrected calcium (pcCa) concentrations, excluding those with multiple myeloma.
  • Primary outcome assessed was the incidence of life-threatening cardiac arrhythmias and/or neurological complications during emergency department stay.

Main Results:

  • Out of 41,823 patients, 155 had severe hypocalcemia (pcCa <1.9 mmol/L), with 133 analyzed after exclusions.
  • 12.8% of patients with severe hypocalcemia experienced a life-threatening condition (10.5% neurological, 2.2% cardiac).
  • These complications were often explainable by co-morbidities or other electrolyte disturbances, not solely hypocalcemia.

Conclusions:

  • Severe hypocalcemia was associated with life-threatening complications in 13% of emergency department patients.
  • Alternative causes were frequently identified for these complications, suggesting hypocalcemia may not be the primary driver.
  • Further research is necessary to elucidate the precise role and independent contribution of hypocalcemia to critical events.
