    Background:

    • Existing models often assume a linear relationship between nerve fiber diameter and conduction velocity, which may not accurately reflect pathological conditions.

    Observation:

    • CAPs were recorded from the sural nerve in polyneuropathy patients.
    • Initial simulations using a linear diameter-velocity relationship showed systematic deviations from recorded CAPs.
    • Morphological data from nerve biopsies were correlated with electrophysiological recordings.

    Findings:

    • Abandoning the linear diameter-velocity assumption and optimizing the fiber velocity distribution allowed for accurate reconstruction of recorded CAPs.
    • The refined model successfully captured slow components and polyphasic potentials, even in cases of severe fiber loss.
    • A method was developed to empirically estimate the diameter-velocity relationship using biopsy-derived diameter histograms and optimal velocity distributions.

    • This improved modeling approach enhances the diagnostic capabilities for various polyneuropathies.
    • The findings provide a more accurate method for correlating nerve morphology with electrophysiological function.
