    Summary

    A rare case of congenital thoracic kidney was found in Nigeria. This condition, though uncommon, should be considered for lung base masses, with intravenous urography being diagnostic.

    Background:

    • Congenital anomalies of the kidney and urinary tract (CAKUT) represent a significant portion of pediatric surgical cases.
    • Ectopic kidneys, particularly intrathoracic locations, are exceedingly rare congenital malformations.
    • Early identification is crucial for appropriate management and to prevent misdiagnosis.

    Observation:

    • A case of congenital ectopic intrathoracic kidney was identified at the University College Hospital, Ibadan, Nigeria.
    • The patient presented with a mass at the lung base, initially detected via chest radiography.
    • The ectopic kidney was located within the thoracic cavity.

    Findings:

    • Intravenous urography confirmed the diagnosis of intrathoracic kidney.
    • The identified kidney was asymptomatic and demonstrated normal function.
    • No immediate treatment was indicated for the incidentally discovered ectopic kidney.

    Implications:

    • Intrathoracic kidney should be included in the differential diagnosis for lung base masses on chest X-rays.
    • Diagnostic imaging, specifically intravenous urography, can effectively identify this rare condition.
    • Recognizing asymptomatic, normally functioning ectopic kidneys can prevent unnecessary invasive investigations and surgical interventions.
