The uterine or fallopian tubes function as the conduit through which oocytes travel from the ovaries to the uterus.     Summary

    Uterine leiomyosarcoma (uLMS) is a rare, aggressive cancer often mistaken for fibroids. Accurate pre-operative diagnosis is crucial for effective surgical treatment and improved patient prognosis.

    Background:

    • Uterine leiomyosarcoma (uLMS) is a rare gynecologic malignancy with a poor prognosis, primarily affecting pre- and peri-menopausal women.
    • Accurate pre-operative diagnosis of uLMS is challenging due to its similar presentation to benign uterine fibroids.
    • Current treatments for uterine fibroids, such as morcellation, can worsen the prognosis of undiagnosed uLMS.

    Observation:

    • uLMS is frequently diagnosed incidentally after surgery intended for presumed benign conditions.
    • The prognosis of uLMS is significantly better for early-stage tumors treated without uterine injury.
    • Adjuvant systemic treatments and radiotherapy have not demonstrated proven value in improving outcomes for uLMS.

    Findings:

    • There is a critical need for a reliable pre-operative risk score to guide diagnostic strategies and surgical planning for uLMS.
