    Summary

    This study presents a modified surgical technique for primary lymphedema using an omental flap. The combined enteromesenteric bridge and omental flap procedure effectively improved lymphatic drainage and reduced swelling in a young patient.

    • An 18-year-old patient presented with severe lower limb and genital lymphedema, including ulcerative, necrotic, and inflammatory lesions.
    • Imaging revealed bilateral hypertrophic inguinal and external iliac lymph nodes, indicative of proximal lymphatic occlusion.
    • The patient underwent a combined enteromesenteric bridge and omental transposition procedure for physiologic lymph drainage.
