Purpose of the Study:

  • A cohort of 282 ulcerative colitis patients who underwent restorative proctocolectomy between 1985 and 2009 were analyzed.
  • Long-term functional outcomes were assessed using the Öresland questionnaire (0-15 score), with <8 indicating good function.
  • Median follow-up was 13 years (range 4-28).

Main Results:

  • The mean functional score was 5.5, with 70% of patients reporting a well-functioning pouch.
  • Pouchitis occurred significantly more often in patients with poor functional results (51.0% vs. 25.6%, p=0.001).
  • No association was found between functional score and time since surgery; pouchitis was the only significant factor linked to poor function.

Conclusions:

  • Restorative proctocolectomy generally provides good and stable long-term functional results for ulcerative colitis patients.
  • Pouchitis is a significant negative predictor of functional outcomes.
  • Careful pre-operative planning and counseling are essential, particularly for elderly patients.
