One of the common DNA damages is the chemical alteration of single bases by alkylation, oxidation, or deamination. The altered bases cause mispairing and strand breakage during replication. This type of damage causes minimal change to the DNA double helix structure and can be repaired by the base excision repair (BER) pathways. Summary

A rare Bednar tumour, a pigmented variant of dermatofibrosarcoma protuberans, was diagnosed in a patient with prior melanoma. This case highlights the importance of differentiating pigmented skin lesions, especially near melanoma scars.

Background:

  • Melanoma is a significant skin cancer with potential for metastasis.
  • Dermatofibrosarcoma protuberans (DFSP) is a rare, locally aggressive skin sarcoma.
  • Bednar tumour is an uncommon pigmented variant of DFSP.

Observation:

  • A 74-year-old man presented with a slow-growing mass near a prior melanoma excision scar.
  • Initial clinical diagnosis suggested melanoma metastasis.
  • Histopathology revealed a dermal spindle cell proliferation with pigment-containing dendritic cells.

Findings:

  • Immunohistochemistry confirmed spindle cell positivity for CD34 and vimentin.
  • Dendritic cells stained positive for S100, Melan-A, and HMB-45.
  • A final diagnosis of Bednar tumour was established.

Implications:

  • Bednar tumour requires careful differential diagnosis from other pigmented skin neoplasms.
  • The proximity to a melanoma scar raises questions about the origin of pigmented cells.
  • Understanding Bednar tumour's characteristics is crucial for accurate diagnosis and patient management.
