    Background:

    • Hepatocellular carcinoma (HCC) is a primary liver malignancy.
    • Pedunculated HCC is a rare subtype with potential for extrahepatic growth.
    • Understanding the characteristics of rare HCC variants is crucial for diagnosis and treatment.

    Observation:

    • Reported two cases of pedunculated hepatocellular carcinoma.
    • One case had a known 5-year history.
    • Both patients survived post-surgery (1 and 2 years), with one developing bone metastasis.

    Findings:

    • Pedunculated HCC shares similarities with general HCC, despite minor differences.
    • Slow growth and favorable prognosis are linked to its extrahepatic location.
    • Extrahepatic origin may stem from accessory liver lobes.

    Implications:

    • Pedunculated HCC may have a better prognosis than typical HCC.
    • Surgical intervention can be effective for pedunculated HCC.
    • Further investigation into the etiology of accessory lobe-derived HCC is warranted.
