    Conventional ventilation fails for acute respiratory failure in unilateral lung disease. Asynchronized differential lung ventilation offers a therapeutic solution for severe hypoxemia caused by intrapulmonary shunting.

    Background:

    • Acute respiratory failure (ARF) in unilateral lung disease poses challenges with conventional ventilation.
    • Increased intrapulmonary shunt and reflex vasodilation in the injured lung contribute to poor therapeutic response.

    Observation:

    • Patients with unilateral lung disease showed deteriorating respiratory parameters despite conventional mechanical ventilation.
    • Reduced functional residual capacity and decreased compliance in the injured lung led to gas flow deviation and worsening hypoxemia.

    Findings:

    • Asynchronized differential lung ventilation (ADLV) proved effective in two cases of unilateral lung disease unresponsive to conventional methods.
    • ADLV addresses the pathophysiology of unilateral lung injury by optimizing ventilation to each lung independently.
