Background:

  • Staphylococcus aureus bacteraemia is a common and often fatal infection.
  • Adjunctive rifampicin is hypothesized to enhance bacterial killing and reduce mortality.

Purpose of the Study:

  • To determine if adjunctive rifampicin reduces bacteriological failure/recurrence or death in S. aureus bacteraemia.
  • To evaluate the impact on mortality, toxicity, resistance, and cost-effectiveness.

Main Methods:

  • A parallel-group, blinded, placebo-controlled multicentre trial.
  • Adult inpatients with S. aureus bacteraemia received standard antibiotics plus either rifampicin or placebo for 14 days.
  • Primary outcome was all-cause bacteriological failure/recurrence or death at 12 weeks.

Main Results:

  • No significant difference in the primary outcome (16.8% rifampicin vs. 18.3% placebo).
  • Bacteriological recurrences were lower in the rifampicin group (0.8% vs. 4.1%, p=0.01).
  • Increased adverse events and drug interactions were observed with rifampicin.

Conclusions:

  • Adjunctive rifampicin offers no overall benefit over standard antibiotic therapy for S. aureus bacteraemia.
  • Further research into alternative antibiotic combinations or source management is warranted.
