- Primary hyperparathyroidism (PHPTI) is rare, typically caused by parathyroid adenoma, hyperplasia, or tumor, leading to parathyroid hormone (PTH) oversecretion.
- Respiratory symptoms are an uncommon presentation of PHPTI.
- Diffuse metastatic pulmonary calcification (MPC) is not a characteristic finding of PHPTI.
Observation:
- A 49-year-old female presented with a month of fever, cough, and exertional asthma.
- Imaging revealed diffuse high-density shadows in both lungs, consistent with interstitial inflammation and calcification.
- A large right parathyroid adenoma was identified via ultrasonography and confirmed with MIBI imaging.
Findings:
- Computed tomography (CT) and lung biopsy confirmed interstitial lung disease with calcification.
- Parathyroid hormone (PTH) oversecretion due to a parathyroid adenoma led to PHPTI.
- The patient underwent successful resection of the parathyroid adenoma.
Implications:
- This case is the first to report PHPTI presenting with respiratory tract infection and diffuse MPC.
- The findings may enhance clinical recognition of PHPTI in patients with unexplained respiratory symptoms and pulmonary calcification.
- Early diagnosis and intervention for PHPTI can alleviate respiratory complications.
