This study validated a rapid algorithm for detecting carbapenemase-producing Enterobacteriaceae (CPE). The algorithm shows high sensitivity and negative predictive value, making it suitable for routine clinical screening.
Area of Science:
- Clinical microbiology
- Infectious diseases
- Diagnostic algorithm development
Background:
- Carbapenemase-producing Enterobacteriaceae (CPE) pose a significant global health threat.
- Rapid and accurate detection of CPE is crucial for infection control.
Purpose of the Study:
- To prospectively evaluate the analytical performance of a novel CPE detection algorithm.
- The algorithm combines phenotypic screening with MALDI-ToF hydrolysis for confirmation.
Main Methods:
- A 6-month prospective study included 74 carbapenem-resistant bacterial strains.
- Phenotypic screening followed by MALDI-ToF hydrolysis for suspected CPE.
- Molecular methods used for reference confirmation.
Main Results:
- The algorithm demonstrated 100% sensitivity and negative predictive value.
- Specificity was 77%, with a positive predictive value of 20%.
- Three out of seven initially positive strains were confirmed as false positives by molecular testing.
Conclusions:
- The algorithm offers a 24-hour turnaround time for CPE detection.
- It effectively reduces the need for costly molecular tests in routine screening.
- The algorithm is recommended for routine screening of clinical strains.
