Background:

  • Chronic lung allograft dysfunction (CLAD) is a major barrier to long-term survival after lung transplantation (LTx).
  • Understanding CLAD phenotypes and their underlying causes is crucial for improving patient outcomes.
  • A recently proposed classification system for CLAD requires validation.

Purpose of the Study:

  • To evaluate the prevalence of obstructive and restrictive CLAD phenotypes.
  • To assess the prognostic importance of these CLAD phenotypes.
  • To validate a new classification system for CLAD.

Main Methods:

  • Retrospective analysis of double LTx patients (2004-2015) with ≥180 days survival.
  • CLAD defined by a persistent ≥20% decline in forced expiratory volume in 1 second (FEV1).
  • Patients classified by obstructive (FEV1/FVC < 70%) or restrictive (TLC ≤ 90% or FVC ≤ 80%) patterns and known/unknown causes (BOS/RAS).

Main Results:

  • CLAD developed in 39% of patients; 20% had an identifiable cause.
  • Restrictive CLAD (26% survival) had significantly worse survival than obstructive CLAD (64% survival).
  • Non-BOS patients had inferior survival compared to BOS patients (p=0.0016).

Conclusions:

  • Accurate diagnosis of CLAD phenotypes is critical for patient prognosis.
  • Phenotype classification aids in understanding pathogenesis and guiding therapeutic strategies.
  • Identifies homogenous patient groups for clinical trials.
