Summary

This study found that higher posttraumatic stress disorder (PTSD) symptom severity in veterans was linked to increased white matter organization in the left cingulum angular bundle (CAB). Further research is needed to understand this connection.

Background:

  • Most prior studies utilized region-based analyses, limiting comprehensive understanding.
  • A tract-based approach offers a more detailed investigation of white matter in PTSD.

Purpose of the Study:

  • To investigate the relationship between PTSD severity and white matter fractional anisotropy (FA) using a tract-based approach.
  • To address limitations of region-based analyses in prior PTSD white matter studies.

Main Methods:

  • Diffusion and structural MRI scans were acquired from 67 combat-exposed US Veterans.
  • Tract-based analysis (TRActs Constrained by UnderLying Anatomy) was employed.
  • Partial correlations examined PTSD severity and FA in the cingulum and uncinate fasciculi, controlling for covariates.

Main Results:

  • A significant positive correlation was found between FA of the left cingulum angular bundle (CAB) and PTSD symptom severity (r = 0.433, p = 0.001).
  • This finding remained significant after Bonferroni correction.
  • No significant correlations were observed for the uncinate fasciculi.

Conclusions:

  • Increased organization of the CAB may be associated with greater PTSD symptom severity.
  • The CAB's connections to the default mode network (cingulate cortex, hippocampal subiculum) may explain observed network alterations in PTSD.
  • Further research on CAB integrity is warranted to understand its role in PTSD pathophysiology and cognitive deficits.
