Summary

The left atrial appendage (LAA) may initiate atrial fibrillation (AF). Radiofrequency LAA isolation restored sinus rhythm in the atria, but AF continued within the LAA itself.

Background:

  • The left atrial appendage (LAA) is increasingly recognized as a potential source for atrial fibrillation (AF) initiation and maintenance.
  • Understanding the role of the LAA in AF pathophysiology is crucial for developing effective treatment strategies.

Observation:

  • This study investigated the electrical activity of the LAA during and after radiofrequency (RF) ablation procedures.
  • Radiofrequency wide-area isolation aimed to electrically isolate the LAA from the left atrium.

Findings:

  • Restoration of sinus rhythm was observed in the main atrial chambers following LAA electrical isolation.
  • Despite successful isolation of the LAA from the atria, persistent AF was documented within the LAA itself.

Implications:

  • These findings suggest that while LAA isolation can help control AF in the atria, the LAA may harbor independent arrhythmogenic substrate.
  • Further research is needed to fully elucidate the role of the LAA in AF maintenance and to optimize ablation strategies targeting this structure.
