    Background:

    • Neuralgic amyotrophy is an idiopathic syndrome characterized by limb weakness and muscle wasting, typically affecting the upper brachial plexus.
    • The exact cause remains unclear, but it is often associated with nerve damage.

    Observation:

    • Seven patients presented with cranial nerve involvement (facial, spinal accessory) during typical neuralgic amyotrophy attacks.
    • Some patients experienced recurrent brachial plexopathies or combined brachial and lumbosacral plexopathies.

    Findings:

    • An underlying demyelinating neuropathy was identified in one patient.
    • Two patients with these extended neurological symptoms were identified as heroin addicts.

    Implications:

    • These cases suggest neuralgic amyotrophy can be part of a more extensive peripheral nervous system disorder.
    • The findings provide indirect evidence supporting a systemic immunological factor in the pathoetiology of neuralgic amyotrophy.
