    Radiation therapy after breast cancer surgery can cause long thoracic nerve palsy. This case report details a patient who developed this condition post-lumpectomy and axillary dissection, with near-full recovery in 5 months.

    Background:

    • Radical mastectomy is associated with a 10% incidence of long thoracic nerve palsy.
    • Lumpectomy with axillary dissection is a more recent breast cancer treatment.
    • This procedure is often followed by aggressive radiation therapy.

    Observation:

    • A young woman underwent left breast quadrantectomy and axillary dissection for breast cancer.
    • Following radiation therapy, she developed isolated left long thoracic nerve palsy.
    • Electrodiagnostic studies confirmed the diagnosis.

    Findings:

    • This is the first reported case of radiation-induced long thoracic nerve palsy.
    • The patient experienced almost full recovery within 5 months.

    Implications:

    • Radiation therapy may be an underrecognized cause of long thoracic nerve palsy.
    • Further investigation into the neurotoxic effects of radiation in breast cancer treatment is warranted.
    • This finding may influence future treatment protocols and patient monitoring.
