Structure-Activity Relationships and Drug Design01:28

Structure-Activity Relationships and Drug Design

1.8K
Drug design is a dynamic field that involves discovering and developing new medications based on specific biological targets. This process heavily relies on structure-activity relationships (SAR) and quantitative structure-activity relationships (QSAR) to guide the design and optimization of efficient drugs.
SAR studies the intricate relationship between a drug's chemical structure and biological activity. It focuses on understanding how modifications to a drug's structure can influence...

Antiepileptic Drugs: Potassium Channel Activators01:20

Antiepileptic Drugs: Potassium Channel Activators

689
Ezocgabine or retigabine, an antiepileptic drug of remarkable efficacy, has revolutionized the management of seizures. It is a potassium channel activator, explicitly targeting the family of Q subtype potassium channels. It enhances the transmembrane potassium currents, regulating neuronal excitability. This action stabilizes the resting membrane potential, a pivotal factor in mitigating the hyperexcitability that characterizes epilepsy.
Ezogabine has gained approval as an adjunctive treatment...

Effect of Hepatic Disease on Pharmacokinetics: Active Drug, Metabolite and Fraction of Metabolized Drug01:14

Effect of Hepatic Disease on Pharmacokinetics: Active Drug, Metabolite and Fraction of Metabolized Drug

231
In pharmacotherapy, monitoring drug concentrations is paramount, especially for drugs whose therapeutic effects hinge on both the active compound and its metabolite. Hepatic impairment profoundly influences drug potency by altering liver function. If the drug is more potent than its metabolite, impaired liver function amplifies drug activity due to elevated drug concentration levels. Conversely, if the metabolite holds greater potency, diminished liver function diminishes drug activity by...

Co-activators and Co-repressors02:04

Co-activators and Co-repressors

8.6K
Gene transcription is regulated by the synergistic action of several proteins that form a complex at a gene regulatory site. This is observed in eukaryotes, where the regulation of gene expression is a complex process. Regulatory proteins in eukaryotes can broadly be classified into two types – regulators that bind directly to specific DNA sequences and co-regulators that associate with regulatory proteins but cannot directly bind to the DNA. These co-regulators are further divided into...

tRNA Activation02:26

tRNA Activation

22.9K
Aminoacyl-tRNA synthetases are present in both eukaryotes and bacteria. Though eukaryotes have 20 different aminoacyl-tRNA synthetases to couple to 20 amino acids, many bacteria do not have genes for all of these aminoacyl-tRNA synthetases. Despite this, they still use all 20 amino acids to synthesize their proteins. For instance, some bacteria do not have the gene encoding the enzyme that couples glutamine with its partner tRNA. In these organisms, one enzyme adds glutamic acid to all of the...

Activation Energy01:26

Activation Energy

86.6K
Activation energy is the minimum amount of energy necessary for a chemical reaction to move forward. The higher the activation energy, the slower the rate of the reaction. However, adding heat to the reaction will increase the rate, since it causes molecules to move faster and increase the likelihood that molecules will collide. The collision and breaking of bonds represents the uphill phase of a reaction and generates the transition state. The transition state is an unstable high-energy state...

    Summary

Avelumab plus axitinib significantly improved progression-free survival by 64% compared to sunitinib in advanced renal cell carcinoma patients. This combination therapy also doubled response rates with comparable safety profiles.

    Area of Science:

    • Oncology
    • Immunotherapy
    • Medical Oncology

    Background:

    • Advanced renal cell carcinoma (RCC) presents a significant clinical challenge.
    • Sunitinib has been a standard first-line treatment for advanced RCC.
    • Novel combination therapies are needed to improve patient outcomes.

    Discussion:

    • The combination of avelumab (an immune checkpoint inhibitor) and axitinib (a tyrosine kinase inhibitor) demonstrated superior efficacy in advanced RCC.
    • This combination therapy resulted in a 64% increase in progression-free survival (PFS) compared to sunitinib.
    • Objective response rates were significantly higher with avelumab plus axitinib (51%) versus sunitinib (26%).

    Key Insights:

    • Avelumab plus axitinib represents a new standard of care for first-line advanced RCC.
